All the results will be recorded, not only on a GOS2 form for each resident’s care plan, but also in a Patient Visual Assessment Summary (PVAS).
Shows information about the patient, the type of eye exam and the date it was given. Details of the prescription, any ocular conditions, visual acuities, and notes. Also shows pictures of any frames we have provided, with the details of that frame and the lenses fitted. We leave all our patients a copy of their PVAS when we fit their spectacles. We encourage relatives or carers to attend when we visit, so with this in mind, we provide a ‘letter of information for relatives’ pre-visit for you to pass on. We have found this to be much appreciated by relatives.
Our frames also have the patient’s name engraved on, whether they’re used for distance or reading (if more than one pair is supplied), as well as the year they were supplied.
